Instructions

Please complete the following form:

  • To ensure a faster refund/exchange process, provide us with as much information as possible.

  • Make sure to include either a valid email address, phone number or fax number.

  • We will respond with an RMA # to the email address, phone number or fax number if your request for a refund or refund is within our policy. 

  • Please do not send us back any item without receiving an RMA number as any unauthorized packages will be returned.

  • Authorization numbers will expire 15 days from the date issued.
